Since the related project will end as of 31.07.2023, the mobilities should be planned to be completed before the specified date. Mobilities made after this date are not considered within the scope of the relevant project.
It should include a date range and it should be stated that you are going to the host institution to receive training within the scope of Erasmus+ Staff Mobility for Training / to give lectures within the scope of Erasmus+ Staff Mobility for Teaching.
- Children of martyrs/veterans card copy (If any)
- "Medical report for disability" from the hospitals authorized by Turkish Ministry of Health (If any)
- Staff affected from the earthquake (See: Selecion Criteria) must upload the necessary documents, family information document receiving from E-goverment portal (stating the relationship with the one who resides in the specified cities) and residence certiticate while making application.

Mobility Agreement:
It must be filled on the computer. It should contain your own signature and the signature of the official at the counter institution. The 'Sending Institution' part of the document will be signed by the ITU Erasmus and International Mobility Office after you submit it to our office. It is not mandatory for this document to be wet-signed, scanned copies of signatures and electronic signatures are acceptable.

Erasmus+ Staff Mobility Agreement for Teaching
Assignment Document:
An assignment request should be made with the decision of the University Administrative Board, stating that there will be Erasmus+ Staff Mobility for Training/Teaching activity. This letter should be written to the ITU Rectorate with the signature of the relevant unit chief. We recommend that you include the result of your placement, your Erasmus+ mobility agreement and your acceptance letter in the attachment. Please upload the decision letter from the university administrative board to the system.
Travel Health Insurance: Travel-health insurance covering the period of your mobility and valid in the country to be visited must be provided.
Copy of Bankbook: Photocopy of EURO bankbook opened at Vakıflar Bankası ITU Affiliated Branch
Grant Agreement: We will keep the final version of the document with wet signature.
After submitting your departure documents to the Erasmus and International Mobility Office, you will recive a prepayment of 70% of the total grant.
Documents to be Delivered to the Office after Returning:
Confirmation of Stay Document: Click here for the template. The document must be printed on the letterhead of the counterparty, signed and stamped. Before sending the original of this document to us, please upload a copy to the application portal.
Documents showing the travel dates: Upload the relevant entry-exit pages in the passport and boarding passes to the application portal.
EU Survey: After submitting your return documents to Erasmus and International Mobility Office, it will be sent to your e-mail address via the system. Based on your return documents, and the completion of the survey, you will receive the remaining 30% of your grant.
